Disease Management Programs
Immediate Homecare & Hospice is home to some of the most comprehensive disease management programs in the state. The primary goal of the program(s) is to focus on Individualized strategies to improve adherence to the medical treatment plan. Our Disease Management Programs include:

- The Beat Goes on (Congestive Heart Failure program)

- Fall Risk Reduction Program
- Medication Adherence
All of our programs include a zone chart, detailed package explaining the disease and care plan in detail with visit instructions and helpful information. Every patient opened to our Agency receives a scorecard journal which is a complimentary journal in which patients are able to list vital signs, glucose levels, weights, medical appointments, allergies, emergency contacts, medications and other pertinent medical information. If our "Beat Goes On" patients do not have a scale the agency will provide one. Our goal is to not only provide quality of care but to assist in educating our patients to be in control of their medical conditions.
